Just so I understand the mechanics... in other qep'a'/qepHom vocabulary lists I've seen on the Klingon wiki there are notes labeled "REQUEST". Is this another way of indicating the same {chabal tetlh} wish list?

The notes in [brackets] are part of the original request. They are notes the requester made to help explain what they meant by their request or what they were hoping to get from their request. They are included because they may have informed Dr. Okrand in his answer, though if the notes Dr. Okrand provided conflict with the notes in [brackets], Dr. Okrand's notes are the only ones that are canon.

Those notes in brackets are from the qep'a' booklet and probably originated from the KLI wish list, chabal tetlh.
