Could someone use {qogh} to describe the whole ear, i.e. outer ear *and* inner ear too ?

Yes. Similarly, someone could use {ngay'} to mean “a nice knock-down argument”. It might be better to ask whether someone who used the words that way would be understood. I believe the answer to that question is “No.”
