Where did the Huh? and What? glosses of toH come from? Where did these glosses for va come from, and why are they bracketed?

So far as I know, toH is well!, so! expressing approximately the same as English Aha!, while va is just a shortened form of Qu'vatlh, which is said in moments of extreme anger, but which we don't have a translation for.
