AFAIK never used by Okrand.

(KLS):  Military ranking, usually above captain and below admiral.

(ST Encyclopedia 54):  Title formerly given to high-ranking Starfleet officers such as those in charge of a starbase. 

Occasionally heard in TOS (e.g. Commodore Matt Decker IIRC), the rank fell into disuse in TNG, DS9, and VOY.  Were there any mentioned in ENTERPRISE?
