I’m afraid it’s “b”.  E.g.

  If you are afraid to die, you have already died. (TKW)

“I’m afraid (that)…” is a way of softening a criticism, request, etc. in polite English.  It doesn’t refer to genuine fear in the same way that “I’m afraid to criticize Molor” would, for example -- something you might well say if Molor was glaring at you while holding a knife to your throat.
